Emphysematous Cholecystitis
Overview
Emphysematous cholecystitis is a rare but life-threatening form of acute cholecystitis characterized by the presence of gas within the gallbladder lumen, wall, or surrounding biliary structures. Distinct from gallstone-induced cholecystitis in both pathophysiology and epidemiology, this condition is associated with substantially higher morbidity and mortality and occurs most frequently in individuals with diabetes mellitus or immunocompromising conditions. Prompt recognition and treatment are essential because delayed diagnosis can lead to severe complications and poor clinical outcomes. This course reviews the pathogenesis, risk factors, clinical presentation, diagnostic evaluation, imaging findings, and management of emphysematous cholecystitis. Participants learn to identify characteristic features of the disease, differentiate it from other biliary disorders, and apply evidence-based treatment strategies for timely intervention. Collaboration among clinicians, surgeons, radiologists, nurses, and other healthcare professionals enhances diagnostic accuracy, expedites treatment, improves care coordination, and supports better patient outcomes through comprehensive multidisciplinary management.

Learning Objectives
At the conclusion of this activity, the learner will be better able to:
- Compare the epidemiology, pathophysiology, clinical presentation, imaging findings, and outcomes of emphysematous cholecystitis with those of uncomplicated acute cholecystitis.
- Determine appropriate diagnostic modalities, the urgency of intervention, and management strategies based on the patient's presentation and disease progression.
- Assess patient risk factors, clinical manifestations, comorbidities, and disease severity associated with emphysematous cholecystitis, particularly in individuals with diabetes mellitus or immunocompromising conditions.
- Develop interprofessional communication and collaborative management strategies among clinicians, surgeons, radiologists, nurses, infectious disease specialists, and allied healthcare professionals to optimize care coordination and patient outcomes.
- Outline the pharmacologic therapy as it applies to Emphysematous Cholecystitis.
